    Refractive eye surgery is a type of eye surgery that is used to rectify refractive errors of the eye and decrease dependency on corrective lenses such as eyeglasses and contact lenses. Successful refractive procedures can reduce myopia or nearsightedness, hyperopia or farsightedness, and astigmatism or elongated corneas. A number of different procedures exist for refractive eye surgery depending upon the type and severity of the refractive error. There are four main types of refractive eye surgery procedures: flap and photoablation procedures; corneal incision procedures; thermal procedures; and implants. Currently, the most common refractive eye surgeries involve the use of lasers to reshape the cornea. Flap procedures involve cutting a small flap in the cornea so that the tissue underneath can be reshaped to correct the refractive error. LASIK, short for Laser Assisted In-Situ Keratomileusis, is the most popular refractive surgery and is used to correct myopia, hyperopia, and astigmatism. The LASIK procedure involves using a microkeratome or IntraLase to cut a flap into the stroma, moving the flap out of the way, removing excess corneal tissue with an excimer laser, then replacing and smoothing out the flap. LASEK is best suited for individuals with thin or flat corneas. The LASEK procedure uses a small trephine blade to cut into the shallow epithelium, after which the eye is bathed in a mild alcohol solution to soften the edges of the epithelium. The flap is gently moved out of the way so that an excimer laser can remove excess corneal tissue, after which the flap is replaced and smoothed out. Epi-LASIK, like LASEK, involves a shallow cut into the epithelium, but makes use of epikeratome to create a thin epithelium sheet for removal instead of the harsher blade and alcohol. Photoablation, the second stage in flap procedures, makes use of ultraviolet radiation to remove excess corneal tissue. PRK, or photorefractive keratectomy, was the original laser eye surgery procedure. PRK involves numbing the eye with local anesthetic eye drops, and reshaping the cornea by destroying miniscule amounts of tissue from the surface of the eye. The diamond knife is used to cut incisions that are parallel to the edge of the cornea, as opposed to the spoke-shaped incisions of the RK procedure. These procedures have been much less common with the emergence of laser-assisted refractive eye surgeries. Thermal procedures use heat to correct temporarily hyperopic refractive errors, or farsightedness. The thermal keratoplasty procedure involves putting a ring of 8 or 16 small burns on the eye immediately surrounding the pupil. The application of the heat increases the slope of the cornea, making it steeper, through thermal contractions. There are two main types of thermal keratoplasty. Laser Thermal Keratoplasty, or LTK, is a no-touch procedure that uses a holmium laser. Conductive Keratoplasty, or CK, uses a high-frequency electric probe. The final type of refractive eye surgery involves the use of implants. Implantable contact lenses, or ICL, can be used to correct severe levels of myopia, hyperopia, and astigmatism. The implants are actually tiny contact lenses that are inserted through a small incision in the side of the cornea. Implants are seated so they sit immediately in front of the eye's natural lens just behind the cornea. ICL works in conjunction with the eye’s natural lens to refocus light on the retina and produce a crystal clear image. Each of these procedures has its advantages and disadvantages, and not all individuals are suitable candidates for refractive eye surgery. In order to reshape the nose, the soft flexible cartilage and bone are manipulated into the proper position and shape. Depending on the desired results, tissue may be removed or added to change the angle or contour of the nose. A chisel or file may be used during the procedure, most frequently to remove a hump from the bridge of the nose. Once the nose has been sculpted into the desired shape, the skin is resituated and the nose is splinted to protect it. Nasal pads may be used for the first few days following the surgery to pack the nose and protect the delicate septum. Rhinoplasty, as with all other surgeries, is not risk-free. In addition to the standard bruising and swelling, patients should be aware of a number of potential side-effects that could result from the procedure. These include an infection of the nose, nosebleeds, numbness, scarring, and a reaction to the anesthesia. Some patients report the bursting of small blood vessels on the surface of the skin and hematomas, or collections of blood that pool under the skin. The possibility also exists that a follow-up procedure will be necessary in order to remedy an over-correction or under-correction of the nose.     Rosacea gives dilated blood vessels and inflammation in the skin. The skin is often also infected by bacteria. Most often the face is attacked and especially the convex surfaces of the face. Sometimes also the eyes or other body parts are affected. Rosacea may begin early, but distinct and serious symptoms usually develop as an individual gets older. WHAT SYMPTOMS ARE SEEN BY ROSACEA The primary symptoms of rosacea are the following. All the symptoms may not be present always. - Permanent redness in the skin. This redness can get better or worse periodically. - Flushing or redness that comes and disappears periodically. - Telangiectasia - This is a distinct widening of small blood vessels in an area. - Papules - Small dome-shape spots filled with fluid, often group wise. - Skin areas with rosasea symptoms often get infected, and the infection worsens the symptoms. - Pustules or pimples – Sometimes infection makes the papules develop into real pimples. However, if the pimples appear in a hair follicle stuffed with sebum (comedones), they are a symptom of acne, and not rosacea. Pimples from acne and rosacea may exist together. Rosacea also implies secondary symptoms that are caused by the primary symptoms, or appear later than the primary ones. - Burning or stinging - Placks - elevated red areas - Scaling, often associated with burning or stinging - Dry skin, often occurring before scaling - Edema coming in areas that already has been red for some time - Symptoms from the eyes like: Redness, itching, burning, corneal damage - Thickening of the skin, or lumps in the skin - Distended hair follicles - Rosacea in other places than the face WHAT CAUSES ROSACEA The exact causes are not known, but some people seem to inherit the tendency of developing rosacea. People that blush very often seem to develop rosacea more easily. Therefore rosacea may be caused by blood vessels that dilate too easily upon stimulation, and eventually become permanently dilated. The following types of stimulation may cause worsening of the rosacea symptoms: heat, hot baths, strenuous exercise, sunlight, wind, very cold temperatures, hot or spicy foods and drinks, alcohol consumption, menopause, emotional stress, prolonged use of topical steroids on the skin. HOW IS ROSACEA TREATED Traditional medicine does not have any treatment for the basic processes of rosacea, such as redness and the blood vessel extention. Laser treatment may be used to take away dilated blood vessels or skin irregularities by rosacea. The laser light heats and destroys the dilated blood vessels. When infection is a part of the disease, and papules or pustules occur, rosacea is treated with antibiotics, usually applied directly on the skin, for example metronidazole. Oral treatment may also be used by severe rosacea, for example with: Tetracycline, minocycline, erythromycin, and doxycycline. By pimple formation, you can also use rinsing skin cleansers in the same way as by acne. On the market you can find several alternative medicines, mostly topical creams, to treat rosacea. These often contain natural oils, herbal extracts, vitamins, minerals and anti-oxidants that alleviate inflammation and stimulate skin healing. They may also contain mild antiseptic agents to treat bacterial infection.

    Why do I have sciatica? Is there successful treatments available? Most work-related sciatica injuries are caused or aggravated by actions such as heavy lifting, vibration, repetitive motion, and awkward posture. Sciatica usually is derived from a traumatic event, muscle disproportion, or a combination of both. Sciatica soreness is caused by force on the sciatic nerve, normally caused by a tightening and shortening of the piriformis muscle. This is more often than not produced by a disproportion between the inner and outer hip rotator muscles. The ordinary low back troubles and other spinal situations that can cause sciatica - pain along the sciatic nerve - include: 1). Lumbar Herniated Disc. A herniated disc happens when the inner core of the disc breaks through the fibrous outer core of the disc and the bulge places force on the nearby nerve root as it exits the spine. In general, it is said that a sudden twisting movement or injury can lead to herniation and sciatica. But, most discs get worse due to recurring stress and then herniation. A herniated disc is usually referred to as a slipped, ruptured, bulging, or protruding disc...or a pinched nerve. 2). Lumbar Spinal Stenosis. This usually causes sciatica due to a tightening of the spinal canal. It typically results from a grouping of one or more of the following: enlarged facet joints, overgrowth of soft tissue and a bulging disc placing force on the nerve roots as they exit the spine. 3). Isthmic Spondylolisthesis. This condition rarely causes back pain or sciatica pain. Caused by a combination of disc space collapse, the fracture, and the vertebral body slipping ahead, the L5 nerve can get pinched as it exits the spine. 4). Piriformis Syndrome. The sciatic nerve can also get aggravated as it runs under the piriformis muscle in the rear. If the piriformis muscle irritates or pinches a root that includes the sciatic nerve, it can cause sciatica-type pain. L4 and L5 are the most common to be fused) and held in place with titanium screws and either rods or a plate. The surgery can take 4 hours and is a very painful experience. The spinal fusion can take 6-12 months for recovery. The pain associated with the 1st month of recovery is quite extreme and will require mobility assistance, physical therapy and the use of a walker. It is important to note that no surgical procedure is 100% successful in releaving all of the patients symptoms. In addition, each tablet contains as inactive ingredients: carboxymethylcellulose calcium, magnesium stearate, mannitol and starch. Does Tamoxifen Influence Female Hormones? Tamoxifen blocks the female sex hormone oestrogen. The hormone influences the growth of cells related to female reproduction, such as those in the breast or the uterus. If there is too much oestrogen in the system, cell growth can accelerate to the point where tumours start to develop. Tamoxifen competes with the sex hormone for the same proteins - called receptors - found on the surface of cancer cells. When the drug locks onto the receptors it blocks the way for oestrogen - which would otherwise activate the cancer cell to divide and make the tumour grow. FDA Regulations on Tamoxifen On October 29, 1998, the Food and Drug Administration approved Nolvadex (tamoxifen citrate) for reducing the incidence of breast cancer in women at high risk for developing the disease. This new indication for tamoxifen, which has been used as a breast cancer treatment for more than 20 years, resulted from a recent study of the drug, conducted by the National Cancer Institute (NCI), in women who were judged to be at increased risk of breast cancer. The study showed that tamoxifen reduced the chance of getting breast cancer by 44 percent. Scientists studying cognitive control signals report that “eavesdropping” on neurons in the brain allow them to predict how the body will respond. The signals can be directed to a prosthetic brain, or electrical processing center, which interprets the signals and moves the prosthetic limb. For example, consider the individual who goes to reach for a book. As he decides to reach, he sends an invisible electrical signal to the brain that tells the body it needs to prepare to move. The brain receives the signal and decodes it, sending a signal to the right arm to extend and move towards the book. Scientists propose using these same mechanics to develop smart prosthetic devices. This entails tapping into neural signal pathways and using them to direct prosthetic limbs just as the brain directs natural limbs. Sensor sockets are another new innovation in the field of prosthetic development. Sensor sockets are monitoring devices designed to serve as an interface between the limb and the prosthesis. The socket is comprised of a net of sensors that can detect information from the limb and transmit it directly to a medical facility for evaluation. The hope is that the information received from the socket sensor will allow rehabilitation doctors to evaluate the individual’s condition and the performance of the limb. With time, doctors hope that the use of sensor sockets will be used as a means to maximize the effectiveness and comfort of the limb. The sensors could potentially allow for real-time adjustments in the performance of the prosthetic, thereby improving a patient’s quality of life and enhancing patient treatment. In addition to these two exciting new emerging technologies, progress continues to be made in the comfort and construction of current prosthetic devices. Incremental improvements in the flexibility, cushioning, and rotational ability of modern artificial limbs increase a prosthetic’s comfort and functionality, allowing prosthetic wearers to come closer to feeling like they are wearing a natural limb. And as the quality of prosthetics increases, prosthetic wearers are more able to resume activities previously made difficult by their lack of a natural limb.

A wall made out of rubber is placed on the teeth and the laser is directed towards the teeth and the heat generated enters the teeth through the solution and the treatment takes an hour or so. After the treatment is completed you can see the changes. 3) Whitening Toothpastes: These toothpastes have special chemicals that remove the stains. The main advantage of the whitening toothpaste is it does not change the natural color of the teeth. Colgate Simply White Advanced Whitening Toothpaste is recommended. 4) Tooth whitening strips: These strips are placed on the teeth and are removed after 30 minutes. These are very effective and are recommended. They also contain peroxides. If the above mentioned ways of treatment are done properly then there won’t be any problem. There is no long-term data on safety of these tooth-whitening treatments.

    Traditional Chinese medicine is gaining popularity in the U. S., as more and more people are learning the benefits of acupuncture, acupressure, deep massage, Qigong, and the herbal medicines. Westerners are the new kids on the block when it comes to receiving these treatments, as over a quarter of the population of the world practices TCM, also known as Oriental medicine. The National Institute of Health and the World Health Organization believe TCM to be a viable alternative to Western medicine, and now many health insurance companies will cover acupuncture as a treatment for various ailments. Very exciting for those of us who find acupuncture to be a great help in lessening pain and improving circulation. Acupuncture originated in China around 3,000 years ago and is the most common form of medical procedure in the rest of the world, because the same treatment not only relieves immediate pain, it is a preventative procedure because it realigns the Qi ( pronounced "Chee.") The Qi is the body's vital energy, all things rise from Qi and it flows and connects a person to the earth and sky. It is also the source of the body's natural healing, all disorders can be traced to a Qi imbalance, and the acupuncture points on the body release the bound Qi and allow it to flow naturally-the body can then begin to heal itself. Diagnosis in acupuncture involves the practitioner asking a long list of questions, some usually considered by Americans to be of a private nature. The questions consist of everything from listing sleeping patterns to a typical day meals to how many times the patient has a bowel movement in a 24 hour period. These are necessary questions, as the more information the acupuncturist has, the better to tell where the Qi is blocked. The patient typically will then lie down on a massage table (in the East acupuncture is usually done in the sitting position) and the practitioner will insert very thin needles into some of the 2,000 plus points in the body which connect to 14 pathways, or medians. The insertion of the needles does not hurt, and contrary to what you may have heard, he/she does not twist or bore them into the skin. They are inserted just underneath the surface and the patent is left in a soothing, dimmed room for about an hour. There are different sizes and lengths of needles, each for a specific ailments and point. The needles are made of super thin stainless steel and are pliable; they are discarded after each use. Another form of TCM is herbal remedies; these are often used in conjunction with acupuncture. Because all things are interconnected, pains and conditions in the body have a lot to do with one's emotions and thinking process. That is why people who experience chronic pain are also frequently depressed. TCM practitioners will often give the patient herbs to help relax or calm them along with the acupuncture sessions. This is called tonic herbalism, and some popular herbs used are probably already in your kitchen. Green tea and ginseng are two of the most well known herbs used in TCM, but here are some others-considered ''warm" foods which will help maintain good health and restore balance to a depleted Qi: Rice and noodles, broccoli, egg whites, fish and chicken. It is no coincidence that these are some of the main ingredients in all Eastern cooking. In order to get a holistic view of TCM, we have covered acupuncture (prevention and treatment), herbal remedies and diet, it is only right that we should touch on exercise. Enter Qigong (Pronounced Chee Kung.) Qi, of course, still means energy, and gong means work or practice. The art and practice of Qigong involves practicing posture, breathing, meditation, and slow regulated movement of the limbs. Qigong is a great exercise, and because of its popularity, you can usually find classes in your area simply by opening the phone book.

    Tumescent liposuction involves the surgical removal of excess body fat using a combination of traditional liposuction techniques and tumescent anesthesia. The procedure is relatively new to the cosmetic surgery scene, having come into widespread use in the 1990’s. Tumescent liposuction offers significant benefits over traditional liposuction techniques for its ability to minimize post-operative discomfort, curtail surgical bleeding, and reduce bruising and swelling during the recovery period. Tumescent liposuction is unique in that it uses large volumes of a dilute solution of lidocaine, paired with the drug epinephrine. The lidocaine serves as a local anesthetic, eliminating the need for patients to go under general anesthesia. The epinephrine serves to temporarily restrict capillaries, thereby reducing bleeding during the surgery. This combination allows surgeons to perform liposuction using minimal amounts of sedatives. The lidocaine is retained in the body for several hours after the procedure, giving the patient up to 12 to18+ hours of pain-free recovery. The use of tumescent anesthetic also eliminates the drunken feeling felt by many patients after going under general anesthesia, as well as the nausea felt by traditional liposuction patients. At the start of the procedure, the surgeon anesthetizes the skin at select locations using tiny needles. Once the skin has been numbed, longer needles are used to anesthetize the subcutaneous fat with large quantities of the dilute lidocaine and epinephrine solution. Patients are frequently given a mild sedative, such as Valium to help relax them during the procedure. Once the areas to be treated are completely numb, the doctor begins the liposuction. Small holes, called adits, are made in the skin using round skin biopsy punches to form access sites for the liposuction microcannulas. A microcannula is a very small stainless steel tube, ranging from 1mm to 3mm in diameter, designed to effectively remove small amounts of fat. Fat is suctioned out of the body in small tunnels through the microcannula, leaving the fibrous strands attached to the skin and muscles intact. Once the desired fat has been suctioned out, the microcannulas are removed. Since the adits are such small holes, there is no need for sutures using this method. This actually serves to speed up the healing process since drainage can occur via the adits, greatly reducing the amount of swelling and bruising of the treated areas. Tumescent liposuction, as with any other type of surgical procedure, does carry some risks. Patients should expect some side-effects from the procedure such as bruising, swelling, and temporary numbness of the treated areas. Significant drainage will occur in areas where adits were formed. Some skin irregularities may result from the procedure, though this side-effect is greatly reduced with the use of both the tumescent anesthesia and microcannulas. Other, more serious side-effects can include blood clots, infection, and allergic reactions to the anesthesia.

Omega-3, St. John’s wort and SAM-e are popular natural supplements reported to improve the effects of bipolar disorder. Often doses and types of medication will have to be adjusted throughout the life cycle of the disorder for maximum benefit. Psychotherapy is an important treatment and should not be ignored even if medications are prescribed. Individual and group counseling is readily available from psychiatrists and hospitals. It will provide a supportive environment to discuss issues and come to terms with the issues. Education will assist in recognizing signs and improving means of dealing with the symptoms. Including family members as a part of the therapy will help them understand and better cope with the effects of the problem.
